Hi Will,
Have you worked through the MqlWrite tutorial here?
http://www.freebase.com/view/guid/9202a8c04000641f800000000544e111
Generally I create types using the Freebase.com UI and then create
_topics_ programatically. Do you really want to create types using
MqlWrite? (If so then somebody else will need to detail the steps
involved)

Users can't create objects like /test/.... because you don't have
permissions in the root domain. Maybe you wanted to create "test" or
"note" inside /user/wf/default_domain ?
Here is some documentation on Namespaces: http://www.freebase.com/view/guid/9202a8c04000641f800000000544e111#id2625941
By the way, if you include links with your questions it helps us to
investigate:
http://sandbox.freebase.com/tools/explore/user/wf/default_domain/x_test_note
http://tinyurl.com/5acw72 (Using TinyUrl to link to a problematic query)
(I'll pass on questions 2 and 3)
Hope this helps,
