"Story by" in films is pretty standard, I think. It can be used when a film
is adapted from a work in another medium, but it is also used for original
work (one example is The Empire Strikes Back -- story by George Lucas,
screenplay by Leigh Brackett and Lawrence Kasdan).  Plays don't really have
a similar concept, and the Adaptation and Adapted Work types should work for
most cases.
